What is hard water?
Hard water is just water that happens to contain calcium and magnesium compounds. The more compounds it has, the harder it is. However, those particular compounds are mainly found in bedrock, so they can only be absorbed into nearby water when it rains and the water percolates through the ground, dissolving some of the rock. As a result, water from locations like wells is typically much harder than lake or river water.
To reduce the hardness of your water, the extra calcium and magnesium compounds need to be taken out, which can be accomplished with a water softener.

How does a water softener make water soft?
Water softeners work to make water soft through the use of an ion exchange. This process involves the use of man-made resin particles that can exchange calcium and magnesium ions with sodium ions. Once the calcium and magnesium ions are fully replaced within the water, the water becomes soft.
